How to create Post Purchase Surveys?

This guide explains how to set up a survey block on thank you or order status pages.





All merchants have access to this feature.



Setting up a survey block allows the collection of user information after a purchase.



This information serves marketing or other operational needs.



The feedback integrates directly onto the Klaviyo customer profile, enabling customized workflow for users.



Creating the Survey Block



Step 1: Open the checkout wizard app.



Step 2: Click on create extensions.

Step 3: Select after checkout under the "where does the extension appear" section. This is because these are post-purchase surveys.



Step 4: Click on continue.



Step 5: Choose where to display the survey block, either on the thank you page or auto status page. For this guide, select a thank you page survey.



Step 6: Click on continue.



Step 7: Select the survey block.



Step 8: Click on create block.



Customizing the Survey Block



Step 1: Name the block and choose the type of questions. For this guide, create a checkbox and add a question, "What did you like most from our store?"



Step 2: Add an option by clicking add option value add option.



Step 3: Fill in the label. This is what the customer sees. Add any label based on the question asked, such as design of the store, product selections, or specific products sold.



Step 4: Customize options based on the question asked.



Step 5: Assign a value to all options. This is the data that appears on the back end.



Step 6: Enable the show other text option to allow customers to input a custom answer.



Step 7: Add more options as needed.



Step 8: Reorder the questions by dragging and dropping.



Step 9: Define rules for who sees the surveys based on cart total, products bought, discount codes used, customer tags, etc. If no rules are set, all customers will see the survey.



Step 10: In the multiple language section, add a title and subtitle to the survey.



Step 11: Customize the back and next button, the thank you message, and other labels.



Step 12: Once all customization is complete, publish the block in the publish block section.



Step 13: Save the block.



Placing the Block on the Checkout Page



Step 1: Go to settings.



Step 2: Click on checkout.



Step 3: Choose to place the block on a live or draft checkout.



Step 4: Change the page where the survey block is placed based on what was created. If a thank you page survey was created, select the thank you page. If an order status page survey was created, select the order status page.



Step 5: Add the app block. The block to add is the custom fleets block. Give it the same location value that was assigned in the app.



Step 6: Drag and drop the block into the desired sections and click save.



Step 7: Preview the block. If it's not visible, clear cookies from the cookies insights data and try again. Alternatively, review the checkout, complete the purchase, and see the survey block on the thank you page.



The setup for the order status page is similar. The only difference is that an order status page extension must be created first. Once the block is created, it must be placed in the order status page editor rather than the thank you page editor.
